Types of Care Offered:
- Wellness and health maintenance
- Sports/school physicals
- Physical examinations and ongoing care
- Medication management
- Diagnosis and management of chronic conditions (high blood pressure, diabetes, thyroid problems, depression)
- Illness care (viruses, infections)
- Minor surgical procedures (removal or biopsy of lesions)
- Referral for testing and specialist medical care
- Lab services
What We Don’t Do:
- Emergency care (chest pain, asthma, respiratory distress, loss of consciousness)
- After hours or weekend care
- Chronic pain management
- Bipolar, schizophrenia, ADHD
- Broken Bones, x-rays or casting
- Hospice or end of life care
- Medical services for infants and children under the age of five (with the exception of immunizations)

Perry Jansen, MD, MPH
Dr. Jansen is a board-certified family physician with over 25 years of clinical practice. He spent 16 years working in Africa with a focus on HIV/AIDS, tuberculosis, and primary care. He also holds a Masters Degree in Public Health and is passionate about serving those in greatest need.
Alivia “Ali” Miller, DNP, FNP-BC
Alivia “Ali” Miller is a board-certified family nurse practitioner who holds a Doctor of Nursing Practice and is dedicated to developing relationships with patients to understand their health needs and goals to collaboratively develop a plan to achieve their optimal state of health. Ali started her nursing career as a graduate from the University of Texas in 2015 by working in the St. Luke’s health care system serving the Treasure Valley. She then obtained her doctorate in nursing at Gonzaga University in 2021. As an Idaho native she enjoys time outdoors, traveling and spending time with family and friends.
Molly Prengaman, FNP
Molly is a board-certified family nurse practitioner dedicated to providing patient-centered care to help her patients achieve their health goals. Molly’s deep roots in Idaho and her commitment to understanding unique needs ensure each of her patients receives the best possible care. Beyond her profession, she enjoys hiking, reading, traveling, and spending cherished moments with family and friends.
