At Southwest District Health, we are dedicated to supporting the health and well-being of our community members. Our Diabetes Prevention Program, endorsed by the Centers for Disease Control and Prevention (CDC), is designed to empower individuals at risk of type 2 diabetes to make sustainable lifestyle changes and reduce their risk of developing this chronic condition. 

What is Our Diabetes Prevention Program? 

Our Diabetes Prevention Program is an evidence-based lifestyle intervention that focuses on helping individuals with prediabetes or at high risk of type 2 diabetes make achievable changes to their diet, physical activity, and overall lifestyle. Developed in collaboration with the CDC, our program provides participants with the tools, resources, and support needed to make lasting improvements to their health. 

How Does it Work? 

Participants in our program meet regularly with trained lifestyle coaches who guide them through a structured curriculum covering topics such as healthy eating, increasing physical activity, stress management, and goal-setting. Through group support and individualized coaching, participants receive the encouragement and accountability they need to succeed on their health journey. 

Who is Eligible? 

Our Diabetes Prevention Program is open to individuals who have been diagnosed with prediabetes or are at high risk of developing type 2 diabetes. This includes individuals with a family history of diabetes, those who are overweight or obese, and adults aged 18 and older.

Participation may be covered by insurance or offered at a low cost.
